Mission Statement

Community Connections SLS, Inc. is a supported living services agency, that was formed to support developmentally disabled adults. We foster each client so they may achieve their individual goals and become a productive member of the community. We are dedicated to assisting each client so they may live independently and safely in their own home. Community Connections works to build the support structure necessary to ensure each client has a quality life with long-term independence. Founded in 1997, Community Connections has clients throughout Napa County. We recognize that persons who receive services from Community Connections have the right to make these choices, as identified in the Lanterman Act:

  • Where to live
  • Who to live with
  • Who to have friendships and relationships with
  • how to spend their time
  • Choices in employment
  • Choices in directing their future


In a funding partnership with the North Bay Regional Center, services provided by Community Connections include, but are not limited to:

  • Assessment of needs
  • Finding a home and maintaining it
  • Household and community safety
  • Obtaining and maintaining adaptive equipment
  • Hiring and training of personal care staff
  • 24-hour emergency crisis and emergency response
  • Assistance with medication and healthcare
  • Transportation and access to community resources
  • Recreational and leisure activities
  • Independent living skills training, i.e. (money management, cooking, shopping, cleaning, personal hygiene, etc.)
